ABSTRACT

Objective:To investigate the effects of shear stress magnitude and exposure time on the damage of blood component erythrocytes and von willebrand factor (VWF) based on microfluidic technology.Methods:A blood shear platform was built based on a microfluidic chip, samples were prepared under different shear stress magnitudes and exposure time lengths, free hemoglobin assay experiments were performed on blood samples, the hemolysis indices of different samples were measured, and the relative molecular masses of different samples of VWF were analyzed by immunoblotting and chemiluminescence imaging.Results:The quantitative relationships between the hemolysis index and the degradation rate of high relative molecular mass VWF with shear stress and exposure time followed the power function model well.Conclusions:The microfluidic experimental platform has the advantages of a precise and controllable internal microenvironment and easy and rapid detection, which can be used for the quantitative study of blood damage patterns.

ABSTRACT

Objective To study von Willebrand factor(VWF) damage based on a novel Maglev Taylor-Couette blood-shearing device. Methods The magnetic levitation (maglev) Taylor-Couette blood-shearing device was designed, and the blood-shearing platform was built. Fresh porcine blood was tested in circulation loop for 1 hour at laminar flow state. VWF damage was assessed by analyzing sample through Western blot and enzyme-linked immunosorbent assay. Results With the increase of exposure time and shear stress, a large number of high molecular weight VWF multimers were degraded into low molecular weight VWF. The maximum rate of degradation was 569%. When the shear stress increased from 18 Pa to 55 Pa, the ratio of VWF-Rco to VWF-Ag decreased from 45.7% to 32.8%. ConclusionsCompared with initial sample, the VWF damage was mainly manifested by the decrease of high molecular weight VWF and the decrease of VWF activity, and VWF-Ag did not change significantly. The novel maglev Taylor-Couette blood-shearing device can quantitatively control the flow parameters (exposure time and shear stress), and be used for blood damage research in vitro, thus providing references for the design and optimization of extracorporeal membrane oxygenation and blood pump.

ABSTRACT

Objective:To explore the effects of Tiaoren Tongdu Acupuncture on motor function and corticospinal tract (CST) remodeling after cerebral infarction. Methods:From February, 2017 to December, 2020, 54 patients with cerebral infarction were randomly divided into control group (n = 27) and acupuncture group (n = 27), each group was divided into subgroups 1, 2 and 3 according to the impairment of corticospinal tract, with nine cases for each subgroup. All the patients received routine medicine, while the acupuncture group received Tiaoren Tongdu Acupuncture, for four weeks. They were assessed with Fugl-Meyer Assessment (FMA) and modified Barthel Index (MBI) before and after treatment, and scanned with diffusion tensor imaging and diffusion tensor tractography, to obtain the fractional anisotropy (FA) and the bilateral FA ratio (rFA). Results:The scores of FMA and MBI, and FA and rFA increased in both groups (t > 2.841, P < 0.05) after treatment, and increased more in the acupuncture group than in the control group (t > 2.140, P < 0.05). Conclusion:Tiaoren Tongdu Acupuncture can promote the recovery of CST to improve motor function for patients with cerebral infarction.

ABSTRACT

Objective To evaluate the damage of von Willebrand factor (vWF) induced by shear stress in BPX-80 centrifugal blood pump, and determine whether it can be used as a reference pump for vWF damage research. Methods An in vitro hemolysis test platform was established according to the ASTM standards. The BPX-80 centrifugal blood pump was tested for 8 hours by using fresh porcine blood. The hemolysis level and vWF damage of hourly blood samples were then evaluated and compared with the static control group. ResultsThe hemolysis index of BPX-80 was stable and low during the test; vWF polymer with high molecular weight had a small amount of degradation, and showed no significant difference compared with the static control group; there was no significant change in the concentration of vWF antigen, which was basically consistent with the trend of the static control group. Conclusions BPX-80 centrifugal blood pump has good blood compatibility and can be used as the reference pump for vWF damage and hemolysis evaluation, thereby providing guidance for the design and optimization of new blood pumps.

ABSTRACT

Parasitic diseases still remain the world's greatest health problems and cause huge economic burden in poor areas. The drugs currently used to treat protozoiases and helminthiases have certain defects, and it is urgent to develop more effective therapeutic drugs for these diseases. Berberine is one kind of important anti-inflammatory agents originally derived from Coptis rhizoma. The derivatives of berberine are obtained by modifying the structural site of berberine. In addition to its anti-inflammatory and anti-microbial activities, berberine and its derivatives also have significant anti-parasitic activity. In this paper, we summarized recent progress in the use of berberine and its derivatives against the infections of protozoa ( Leishmania spp ., Trypanosoma spp. , Toxoplasma gondii, Plasmodium falciparum, and Eimeria tenella) and helminths ( Schistosoma mansoni, Schistosoma. japonicum, Echinococcus granulosus, and Toxocara canis), which may providea useful reference for researchers in this field.

ABSTRACT

Objective:To investigate the regulatory effect of non-specific immunity induced by Trichinella spiralis (Ts) on the immune response of small intestinal tissue of mice infected with Plasmodium berghei (Pb)ANKA. Methods:Thirty-six specific pathogen free female Kunming mice (6-8 weeks old, weighting 18-22 g) were randomly divided into 4 groups according to body weight by the random number table method, including control group, Ts-mono-infected group (Ts group), PbANKA-mono-infected group (Pb group), and Ts + Pb-co-infected group (Ts + Pb group), 9 mice in each group. The mice were fed normal food, water and normal feed. The control group was not given any experimental treatment; the Ts group was infected with 20 Ts larvae orally on the first day of the experiment; the Pb group was infected with 1 × 10 6 PbANKA erythrocytes by intraperitoneal injection on the 9th day of the experiment; the Ts + Pb group was infected with 20 Ts larvae orally on the first day of the experiment, and 1 × 10 6 PbANKA erythrocytes were given by intraperitoneal injection on the 9th day. Mice were sacrificed on 22th day after Ts infection and/or 13th day after PbANKA infection, the morphological changes of peritoneal macrophages in each group were observed by transmission electron microscope; the mRNA expression levels of M1-type macrophage markers [inducible nitric oxide synthase (iNOS) and interleukin-6 (IL-6)], M2-type macrophage markers [mannose receptor C type 2 (Mrc-2) and chitinase-like 3 (Ym1)] in the small intestinal tissue of mice in each group were detected by quantitative real-time PCR (qRT-PCR), and the ratios of mRNA expression levels of M2/M1 macrophage markers were compared. Results:Transmission electron microscope showed that the morphology and structure of peritoneal macrophages in the control were normal; more pseudopodia were observed in the peritoneal macrophages in Ts group; and more pseudopodia and engulfed Plasmodium parasites were observed in the peritoneal macrophages in Pb group and Ts + Pb group. The iNOS (1.000 ± 0.290, 1.277 ± 0.251, 3.088 ± 1.110, 2.604 ± 0.773) and IL-6 mRNA expression levels (1.000 ± 0.393, 2.180 ± 0.629, 1.650 ± 0.612, 3.242 ± 1.780) of small intestinal tissue were compared among the 4 groups, the differences were statistically significant ( F=12.420, 5.270, P < 0.05). Compared with the control group, the iNOS mRNA expression levels in the Pb and Ts + Pb groups were significantly increased ( P < 0.05); compared with the Ts group, the iNOS mRNA expression level in the Ts + Pb group was significantly increased ( P < 0.05). Compared with the control group, the IL-6 mRNA expression level in the Ts + Pb group was significantly increased ( P < 0.05). The mRNA expression levels of Mrc-2 and Ym1 of small intestinal tissue in the 4 groups were significantly different ( F=9.890, 20.500, P < 0.05). The Mrc-2 mRNA expression level in the Ts + Pb group was significantly higher than those in the control, Ts, and Pb groups ( P < 0.05). The Ym1 mRNA expression level in the Pb group was significantly higher than that in the control group ( P < 0.05); the Ym1 mRNA expression level in the Ts + Pb group was significantly higher than those in the control, Ts, and Pb groups ( P < 0.05). The Mrc-2/iNOS, Ym1/iNOS of small intestinal tissue in the 4 groups were significantly different ( F=3.642, 22.360, P < 0.05). The Mrc-2/iNOS in the Ts + Pb group was significantly higher than those in the control and Pb groups ( P < 0.05). The Ym1/iNOS in the Ts + Pb group was higher than those in the control, Ts, and Pb groups ( P < 0.05). Conclusion:The non-specific immunity induced by Ts infection is involved in the regulation of intestinal immune response of mice infected with PbANKA, which may promote M2 polarization of macrophages in the small intestinal tissue.

ABSTRACT

Malignant tumors with multidrug resistance is one of the major reasons for the failure of tumor treatment, and it is also an important issue in the field of cancer treatment. In recent years, with the continuous research on tumor immune microenvironment, the status of tumor immune escape is deeply understood. The close relationship between the abnormal expression of related molecules B7-H1 and tumor resistance is constantly recognized that multidrug resistant tumor may be more sensitive to immune system surveillance and killing than chemotherapy-sensitive tumor. The article reviews the progress of immune escape-related molecule B7-H1 in multidrug resistance of malignant tumors.

ABSTRACT

This study investigated the effect of angiotensin II (Ang II) on apoptosis and thioredoxin-interacting protein (TXNIP) expression in INS-1 islet cells and the underlying mechanism. INS-1 cells cultured in vitro were treated with different concentration of Ang II for different time, and the viability was measured using cell counting kit-8 (CCK-8). After treatment with 1 × 10 mol/L Ang II for 24 h, flow cytometry and Western blot were used to measure the cell apoptosis, and Western blot was used to analyze the protein expression of TXNIP, carbohydrate response element-binding protein (ChREBP) and angiotensin II type 1 receptor (AT1R). Real-time PCR was used to detect TXNIP and ChREBP mRNA expression. IF/ICC was used to observe the TXNIP, ChREBP and AT1R expression. The results showed that Ang II reduced cell viability and induced the expression of TXNIP in a dose- and time-dependent manner (P < 0.05, n = 6) compared with the control group. Ang II induced apoptosis and up-regulated the expression of ChREBP and AT1R (P < 0.05, n = 6). AT1R inhibitor, telmisartan (TM), blocked Ang II-induced TXNIP and ChREBP overexpression (P < 0.05, n = 6) and inhibited Ang II-induced apoptosis. Taken together, Ang II increased ChREBP activation through AT1R, which subsequently increased TXNIP expression and promoted cell apoptosis. These findings suggest a therapeutic potential of targeting TXNIP in preventing Ang II-induced INS-1 cell apoptosis in diabetes.

ABSTRACT

Diabetes can cause a significant increase in the expression of thioredoxin (Trx)-interacting protein (TXNIP), which binds to Trx and inhibits its activity. The present study was aimed to investigate the effect of TXNIP on proliferation of rat INS-1 islet β cells and the underlying mechanism. TXNIP overexpressing adenovirus vectors (Ad-TXNIP-GFP and Ad-TXNIPc247s-GFP) were constructed and used to infect INS-1 cells. Ad-TXNIPc247s-GFP vector carries a mutant C247S TXNIP gene, and its expression product (TXNIPc247s) cannot attach and inhibit Trx activity. The expression of TXNIP was detected by real-time PCR and Western blot. EdU and Ki67 methods were used to detect cell proliferation. Protein phosphorylation levels of ERK and AKT were detected by Western blot. The results showed that both TXNIP and TXNIPc247s protein overexpressions inhibited the proliferation of INS-1 cells, and the former's inhibitory effect was greater. Moreover, both of the two kinds of overexpressions inhibited the phosphorylation of ERK and AKT. These results suggest that TXNIP overexpression may inhibit the proliferation of INS-1 cells through Trx-dependent and non-Trx-dependent pathways, and the mechanism involves the inhibition of ERK and AKT phosphorylation.

Objective:To investigate the effect of up regulation of GDF-15 expression on the proliferation,apoptosis and PI3K/AKT signaling pathway of H9C2 cardiomyocytes induced by H2O2.Methods:CCK8 method was used to detect the proliferation of H9C2 cardiomyocytes treated with different concentrations of H2O2;H9C2 cells were divided into Control group,NC group,H2O2group,GDF-15+H2O2group,the cells were treated for 24 h,mRNA and protein expression of GDF-15 in each group were detected by RT-PCR and Western blot;the proliferation and apoptosis of the cells were detected by CCK8 and flow cytometry respectively;DCFH-DA probe was used to detect the level of ROS;the expression of Ki67,Bcl-2,Bax,PI3K and p-AKT protein was detected by Western blot.H9C2 cells were treated with 10 μmol/L LY294002(a PI3K/AKT signal pathway inhibitor),cell viability and apoptosis rate were detected by CCK8 assay and flow cytometryin espectively.Ki67,Bcl-2,Bax,PI3K and p-AKT protein expression were detected by Western blot.Results:Cell viability was inhibited after different concentrations H2O2treated H9C2 myocardial cells,which was concentration de-pendent (P<0.05),due to H9C2 cardiomyocytes treated with 200 μmol/L H2O2inhibited nearly half of cell proliferation,and were chosen as subjects.Compared with control group,mRNA and protein expression of GDF-15 in H2O2group were significantly increased, cell proliferation was decreased significantly,the apoptosis rate was increased,ROS level was increased,the expression of Ki67,Bcl-2, PI3K,p-AKT protein were decreased,Bax protein expression was increased(P<0.05).Compared with H2O2group,mRNA and protein expression of GDF-15 in GDF-15+H2O2group were significantly increased,cell proliferation was significantly increased,the apoptosis rate was decreased,ROS level was decreased,the expression of Ki67,Bcl-2,PI3K,p-AKT protein were increased,and Bax protein expression was decreased (P<0.05).The cell viability and protein expression of Bcl-2,PI3K and p-AKT in PI3K/AKT inhibitor group were significantly lower than those in GDF-15+H2O2group,and the apoptosis rate and Bax protein expression were significantly higher than those in GDF-15+H2O2group(P<0.05).Conclusion:Up regulation of GDF-15 expression promote the proliferation of H9C2 car-diomyocytes induced by H2O2and reduce apoptosis,and the mechanism may be related to the regulation of ROS levels,Ki67,Bcl-2,Bax expression and PI3K/AKT signaling pathway in cells.

ABSTRACT

OBJECTIVE: Explore the feasibility and safety of transcatheter closure of perimembranous ventricular septal defects using a wire-drifting technique (WT) in children. METHODS: We retrospectively analyzed 121 pediatric patients diagnosed with perimembranous ventricular septal defects who underwent interventional treatment at the First Affiliated Hospital of Xi'an Jiaotong University from Dec 2011 to Dec 2014. Based on the method used for arteriovenous loop establishment during the procedure, the patients were divided into a conventional technique (CT) group and a WT group. RESULTS: In total, 51 of the 53 patients (96.2%) in the CT group and 66 of the 68 patients (97.1%) in the WT group achieved procedural success, with no significant difference between the two groups (p>0.05). The CT group showed a nonsignificantly higher one-time success rate of arteriovenous loop establishment (94.3% vs. 91.2%, p>0.05). The procedure time was 46.0 (14.0) min and 46.5 (10.0) min in the CT and WT groups, respectively. The CT procedure was discontinued in the 2 cases (3.8%) of intraprocedural atrioventricular block in the CT group. In the one case (1.9%) of postprocedural atrioventricular block in the CT group, a permanent pacemaker was implanted to resolve third-degree atrioventricular block three months after the procedure. In the WT group, no cases of intraprocedural atrioventricular block occurred, and one case (1.5%) of postprocedural atrioventricular block occurred. In this case, intravenous dexamethasone injection for three days returned the sinus rhythm to normal. Aggravated mild to moderate tricuspid regurgitation was observed in 2 patients (3.8%) in the CT group during the 2-year follow-up period; aggravated tricuspid regurgitation was not observed in the WT group. During the 2-year follow-up period, there was no evidence of residual shunting in either group. CONCLUSION: Transcatheter closure of perimembranous ventricular septal defects with the WT is safe and effective in children.

Objective To detect the damage of hippocampal neurons and the changes in inflammatory cytokines in rats after cerebral ischemia-reperfusion(I/R)and compare the expressions of IL-1β,IL-6 and TNFαin hippocampal DG,CA1 and CA3 subregions.Methods The focal cerebral I/R model was induced by an intraluminal filament embolism.The SD rats were randomly divided into the sham-operated group(SHAM group)and the middle cerebral artery occlusion-reperfusion group(MCAO group).HE staining was employed to detect the damage to hippocampal DG, CA1 and CA3 subregions.The expression levels of IL-1β, IL-6 and TNFα were detected by immunofluorescence assay.Results Compared with SHAM group,hippocampal DG,CA1 and CA3 subregion neurons in MCAO group were severely damaged, with occurred inflammatory cell infiltration,and a large amount of neurons apoptosis, and the expressions of IL-1β, IL-6 and TNFαin each subregion increased significantly.At the same time, in MCAO group, the expression of inflammatory cytokines in CA1 subregion was more significant than that in DG and CA 3 subregions(P<0.05).Conclusion Cerebral I/R could cause neuronal damage, inflammatory cell infiltration, and neuronal apoptosis in the DG, CA1 and CA3 subregions of the hippocampus and increase the release of inflammatory cytokines.In MCAO group, the expression of inflammatory cytokines in CA1 subregion of hippocampus is significantly higher than that in DG and CA 3 subregions, suggesting that CA1 region is more sensitive to I/R injury.

Objective To evaluate the clinical efficacy and safety of endoscopic full-thickness resection assisted by dental floss traction in treatment of gastric submucosal tumors. Methods Those patients with gastric submucosal tumors from January 2016 to September 2016 were enrolled in the study. All patients were treated with endoscopic full-thickness resection assisted by dental floss traction. The en bloc resection rate, procedure time were analyzed. Complications such as bleeding and perforation were observed in the procedures. All patients were followed up 3-6 months for evaluation of efficacy and safety. Results 96 patients were recruited into the study. The average surgery time of 26.7 min (20 ~ 55 min), the success rate and the en bloc resection rate was 100.0%. The average tumors sizes was 1.2 cm (0.8 ~ 2.5 cm). The intraoperative bleeding occurred in 1 patient, the incidence rate of bleeding was 1.0% (1/96). The tumors were located cardia in 13 patients (13.5%), gastric fundus in 46 patients (47.9%), in gastric body in 33 patients (34.3%), in antrum in 2 patients (2.1%), in gastric angle in 2 patients (2.1%). Pathological results of interstitial tumor in 62 patients (64.6%), leiomyoma in 31 patients (32.3%), calcifying fibroma in 1 patients (1.0%), ectopic pancreas in 2 patients (2.1%). The median hospital stay was 4 days (3 ~ 7 d). No complications such as fever, abdominal pain were found during during the follow-up of 3 ~ 6 months. Conclusion Endoscopic full-thickness resection assisted by dental floss traction in treatment gastric submucosal tumors is effective and safe.

ABSTRACT

Objective To verify the analytical performance of the Roche cobas 8000 automatic biochemical analyzer.Methods The intra-batch and inter-batch precisions,accuracy,linearity and reference interval were validated according to the Clinical and La-boratory Standards Institute (CLSI)documents (EP5-A2,EP15-A2,EP6-A;C28-A2 ).Results The intra-batch precision was 0.5%-2.3% and the inter-batch precision was 0.8%-3.11%,which were all less than 1/4 Tea(CLIA'88).The detection results of various items had better correlation with the target values(r2 >0.99);the linear range of various detected items were in line with the linear range provided by the specification;the quotative biological reference range conformed to the group of this laboratory services.Conclusion The various performance indexes of the Roche cobas 8000 automatic biochemical analyzer conform to the qual-ity requirements and this analyzer can be used in clinical biochemical detection work..

We divided the infertile men into four subgroups according to their perceived causes of infertility: male (M) factors, female (F) factors, both M and F factors, and unknown factors, and compared the SEAR scores among different groups.</p><p><b>RESULTS</b>In the infertile men, the total SEAR score was significantly higher in those with < 3 years than in those with > or = 3 years of infertility duration (75.50 +/- 17.05 vs 68.66 +/- 17.19, P < 0.05) and so was it in those with above-high-school than in those with high-school or lower education background (81.42 +/- 13.99 vs 67.61 +/- 17. 8, P < 0.01), but showed no significant difference between the > or = 30- and < 30-year-olds (71.77 +/- 17.42 vs 72.74 +/- 18.38, P > 0.05). The total SEAR scores in the M factor, M & F factor, unknown factor and F factor groups were 65.69 +/- 18.68, 68.52 +/- 17.68, 74.85 +/- 15.19 and 83.21 +/- 12.61, respectively, with significant differences between the first two and the latter two groups (P < 0.05), as well as between the unknown factor and F factor groups (P < 0.05). In the fertile male controls, the total SEAR score and subdomain scores on sexual relationship, self-confidence, self-esteem and overall relationship were 90.04 +/- 9.85, 88.40 +/- 10.74, 92.23 +/- 9.41, 91.95 +/- 10.67 and 90.38 +/- 14.14, respectively, all remarkably higher than in the M factor, M&F factor and unknown factor groups of the infertile men after adjustment of their infertility duration and education levels (P < 0.05).</p><p><b>CONCLUSION</b>Infertility reduces men's self-esteem, self-confidence, sexual satisfaction and relationship with their spouses, and the degree of its impact is correlated with the patients' infertility duration and education level.</p>

ABSTRACT

Objective To examine the cognitive function of the childhood schizophrenia and healthy children,and to explore the characterized impairment of cognitive function in childhood schizophrenia.Methods Fifty untreated children with first-episode schizophrenia(schizophrenia group) and 50 healthy children(control group) were enrolled in the study.Chinese Wechsler intelligence scale for children (C-WISC),Wiscosin Cards Sorting Test (WCST),Stroop test and Trail making test A and B(TMT-A/B) were used to measure the cognitive function.The Clinical features were assessed with the scale for positive and negative syndrome(PANSS).Results Full intelligence quotient (FIQ),verbal intelligence quotient (VIQ) and performance intelligence quotient (PIQ) in schizophrenia group were significantly lower than those in the control group(P < 0.05),but the change between VIQ and PIQ in schizophrenia group was higher than that in the control group(P =0.023).The complete categories in schizophrenia group were significantly lower than that in the control group(P < 0.05).The total number of errors and the continuous errors in the schizophrenia group were significantly higher than those in the control group(P <0.05).No significant difference of the random errors was observed between the schizophrenia group and the control group.The times to complete word A,color B,double-word C and double-color D in the control group were significantly less than those in the schizophrenia group(P < 0.05).The reaction time of Trail Making Test A/B and the error number of B in the schizophrenia group were significantly higher than those in the control group(P < 0.05).No significant difference of the A error numbers were observed between the 2 groups (P > 0.05).There were significant negative relationships between the negative syndrome and VIQ,PIQ(P <0.05) ;in contrast,there was significantly positive relationship between the negative syndrome and the A error numbers (P < 0.05).There was significantly negative relationship between the total score of PANSS and completed categories (P < 0.05).Multiple regression analysis showed that:there was a significant correlation between the damage of cognitive function in children and negative symptoms,PANSS total score,gender,age,years of education and place of residence (all P < 0.05).Conclusions Children with first-episode schizophrenia have multiple cognitive deficits in speed of processing working memory,spatial memory,attention and executive function.Some of the cognitive impairments have certain relationships with negative syndrome.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the impact of the factors of perceived male infertility on men's penile erectile function.</p><p><b>METHODS</b>Using the 5-item version of the International Index of Erectile Function (IIEF-5), we investigated the penile erectile function among 245 infertile men (infertility group) and another 52 males having physical checkup for planned conception (checkup group), the former further divided into four subgroups according to the factors of infertility: male factor, female factor, bilateral factor, and unknown factor. We compared the total and item scores of IIEF-5 among different groups.</p><p><b>RESULTS</b>Mild or mild-moderate ED was found in 50.61% of the infertile men, with a total IIEF-5 score of 21.24 +/- 2.58. The total and item scores of IIEF-5 were markedly higher in the male factor than in the female factor subgroup (P < 0.01), but with no significant difference between the male factor subgroup and the bilateral and unknown factor subgroups (P > 0.05), except the score on confidence. The scores were significantly lower in the bilateral and unknown factor subgroups than in the female factor subgroup (P < 0.05), with no remarkable difference between the former two. In the infertility group, the total and item scores of IIEF-5 were markedly higher in those with education above high school than in those with high school or lower education (P < 0.01), but not correlated with age and infertility duration. After adjustment for education background, the total IIEF-5 score and the scores on confidence and satisfaction were significantly lower in the infertility than in the checkup group (P < 0.05), and 15.38% of the men in the latter group had mild ED, dramatically lower than in the former (P < 0.01).</p><p><b>CONCLUSION</b>Perceived infertility may cause ED in males, and a higher incidence rate is associated with lower education background of the men.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the characterization of coronary atherosclerotic plaques in patients with unstable angina pectoris (UAP) and stable angina pectoris (SAP) by optical coherence tomography (OCT).</p><p><b>METHODS</b>OCT was performed in 47 patients (23 UAP and 24 SAP) undergoing coronary angiography. Lipid-rich plaque (defined by > or = 2 quadrants of the cross-section area), thin cap fibroatheroma (TCFA), thickness of fibrous cap, plaque rupture, calcification and thrombus visualized by OCT were compared between UAP and SAP patients.</p><p><b>RESULTS</b>OCT imaging was successfully in 44 out of 47 patients (22 UAP, 22 SAP). Proportion of lipid-rich plaques was similar between UAP and SAP groups [91% (20/22) vs. 73% (16/22), P = 0.741]. The minimum thickness of fibrous cap in the UAP group was significantly thinner than that in SAP group [(69.5 +/- 34.7) microm vs. (141.1 +/- 68.5) microm, P = 0.000] and the rate of fibrous cap erosion in the UAP group was significantly higher than that in the SAP group [59% (13/22) vs. 9% (2/22), P = 0.000]. Percents of TCFA [73% (16/22) vs. 14% (3/22), P = 0.000] and plaque rupture [50% (11/22) vs. 9% (2/22), P = 0.003] were significantly higher in UAP group compared those in SAP group. Incidence of thrombus and calcification were similar between two groups.</p><p><b>CONCLUSIONS</b>OCT imaging can clearly define plaque characterization of coronary atherosclerosis. UAP patients have thinner fibrous cap, higher incidences of fibrous cap erosion, plaque rupture and TCFA compared patients with SAP.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyze factors influencing the choice of atrial septal occluder (ASO) for transcatheter closure of patients with secundum atrial septal defect (ASD).</p><p><b>METHODS</b>A total of 1114 ASD patients [388 males, aged from 2 to 75 years, mean age (26.3 +/- 17.0) years] were enrolled. Patients were divided to adult (> 14 years, mean 34.4 years, n = 779) and child (< or = 14 years, mean 7.3 years, n = 335) groups. ASD size in different ultrasound cross-sections was determined by transthoracic echocardiography (TTE). ASO size was chosen on the basis of the maximum diameter of the defect (MD). Defect-shapes and rim lengths of ASD, the difference choice of ASO in the two groups were compared.</p><p><b>RESULTS</b>MD of the defects ranged from 5 to 40 mm [mean (19.7 +/- 7.8) mm]. ASD was successfully occluded in 1085 out of 1114 patients (97.4%). Occluder size ranged from 6 to 46 mm [mean (25.8 +/- 8.9) mm] and the difference between occluder size and MD ranged from 2 to 10 mm [mean (6.1 +/- 3.4) mm, ASO/MD ratio 1.3:1]. Though the diameter of the defect was similar between the 2 groups, the size of occluder was significantly larger in adult group than that in child group (ASO/MD ratio 1.1 - 1.6:1 vs. 1.2 - 1.8:1, P < 0.05). MD was significantly correlated with ASO in both groups (r = 0.911 and r = 0.944 in adults and child groups, respectively, all P < 0.01). The size and increment of the occluder used in patients with deficient anterior rims was significantly bigger than patients with sufficient anterior rims (P < 0.01).</p><p><b>CONCLUSION</b>The maximum diameter of the defect was the major determinant for selecting occluder size and choice of occluder size was also influenced by patient age, defect-shape and defect rim for transcatheter closure of secundum ASD.</p>
